We have a questionaire for you to fill out, including your health history, medical benefits and current areas of pain, etc. This will help us to formulate a plan for your wellness.
When you meet the doctor, we always like to take time out on an initial visit to talk to you about the methods we use and how they can help you regain health, wellness and range of motion.
